The Emergency Information forms provide contact information in case of an emergency and should be carried by the individual at all times.
Individual Report of Hours Donated
ROAM members donate hundreds of hours to churches, camps and special projects. Individuals or couples may use this form for ease in recording the number of hours donated to each project. Team Leaders need totals from these reports for their project report.
Team Leader Report of Hours Donated
The ROAM office reports the total donated hours by ROAM workers to World Mission Mobilization for their annual report. Potential donors to ROAM can readily see the impact ROAM is making in Nazarene camps, churches and special projects.
10% Mission Giving Credit for ROAM Trip Expenses
The Nazarene Global Ministriies Center allows certain travel expenses incurred while working an approved ROAM project to be reported and credited toward the ROAM member's home church for its 10% missions giving. These expenses are to be reported directly to the Work and Witness office on the form provided.
10% Missions Giving and IRS Deduction Instructions
More detailed information about the 10% Missions Giving and expenses that may qualify for an IRS deduction is available for viewing and download. Check with your tax preparer to be sure you qualify for these IRS deductions.
Project Evaluation
Provides feedback evaluation from ROAM members on several aspects of the ROAM project. Provides information to assure that ROAM projects are of high quality to both worker and host.
Buddy Waiver of Liability
Work Voucher
In the event a member has an IRS audit for trip expenses, this form when completed and given to the member before leaving the project validates that the member was working at a particular project at the given time. It is signed by the host manager and the team leader.
